LIST OF DOCUMENTS REQUIRED FOR VISA:

BRAZIL:

• Valid passport with validity of more than 6 month and  Old passport (if any)

• Two recent passport size photos with white background and close-up

• Visa Request form dully filled online from http://novadelhi.itamaraty.gov.br/en-us/visas.xml

• Request letter from applicant mentioning purpose of visit, duration, etc.; and also a declaration stating that it assumes full responsibility regarding the applicant’s business Pursuits& applicant will not exceed legally authorise stay on each visit

• Document related with applicant’s professional status

• If Employee: NOC/leave letter from employer

• If Self Employed: Request letter on Company letter head signed by applicant or any other senior person

• If Student: NOC/Bonafide letter from Institute along with copy if Student ID

• If Minor (not travelling with both parent):ORIGINAL and Notarized copy of Birth Certificate and Notarized letter of consent, signed by the parent not traveling with the minor, or by both parents if the minor is traveling alone (if the minor is traveling with both parents this letter is not necessary if both the parents sign the visa application form, if all names appear on the itinerary and if both parents are applying for the visa at the same time)

• Confirmed return/onward ticket

• Proof of finance – Personal

a) Bank statements for last 3 months certified by bank Manager along with his ID mentioned on it

b) IT papers for last 2 years

c) Pay slips/salary certificate for last 3 month (if employed)

d) Proof of ownership if self employed

e) Proof of finance – Company (if company is responsible for the expenses)

f) Bank statement for last 1 month certified by bank Manager with his ID mentioned on it

g) Income tax papers

h) Registration certificate

i) Photocopy of bio-data pages of passport

CHILE:

1. Valid passport with validity of more than 6 month

2. Recent passport size photo with white background and close-up

3. Visa form to be filled online

4. Covering letter mentioning details of visit, nature of business and declaration about expenses.

5. Confirmed return/onward ticket

6. Authorization in the form of Notarized Affidavit of INR 50 for submission and collection of passport with visa on behalf of applicant. (Please see sample of affidavit)
